Short or retracted columella: evaluating from profile and nostril views

Notes describing a columella that appears retracted or barely visible are illustrated differently across profile and basal views. In one patient, the columella was scarcely visible from the profile view, and basal nostril photos noted an inward-curled columella alongside a depressed scar from a previous open rhinoplasty.

A retracted columella is also documented in primary rhinoplasty patients, meaning this issue cannot be attributed solely to previous surgery. The original notes frequently mention the nasolabial angle, an impression of a protruding mouth, tip drooping, and nostril asymmetry together. Review these cases by looking not only at columellar visibility, but also at its relationship with surrounding structures, surgical history, and findings identified during examination.

Why a nose becomes upturned and the columella retracts after repeated revisions, and how to reconstruct the septum

A case where damage from previous surgeries led to collapse of the anterior septum and nostril asymmetry. The septum was reconstructed using autologous rib cartilage, followed by nasal tip rhinoplasty with donor rib cartilage and implant replacement.

Why implant inflammation recurs, and how staged reconstruction with a resting period is performed

A case of staged nasal reconstruction for a nose damaged by recurrent implant inflammation. After foreign body removal, a 2-year resting period, and stem cell therapy, the structure was rebuilt using donor rib cartilage and an implant was inserted.

Changes that occur when an implant is positioned high and deviated: methods for replacement and bulbous tip correction

A high, deviated silicone implant was replaced to lower the radix, and septal extension was performed using donor rib cartilage. Partial soft tissue resection, cartilage tie, and rhinitis surgery addressed the bulbous tip and nasal obstruction.

Why contracture remains in multiply operated noses: Reconstructing the columella and support framework

Contracture and septal defect resulting from repeated surgeries were reconstructed using autologous rib cartilage. For the retracted columella, retroauricular free skin flap grafting and perichondrium grafting were performed to restore its shape.

Why the nose looks short and the columella retracts after revision rhinoplasty, and how to reconstruct the septum

To correct septal cartilage loss from 3 prior surgeries, a short nose appearance, and a bulbous tip, the septum was reconstructed using autologous rib cartilage and the radix was lowered.

Why a nose looks bulbous or pug-like, and how to refine both the nasal tip cartilage and nasal bones together

A blunt, pug-like nose, retracted columella, and wide nasal bridge were transformed into a slim, three-dimensional profile using autologous rib cartilage tip plasty, columellar lowering, lateral osteotomy, and cartilage reduction with suturing.

Why a crooked nose and dorsal hump appear together, and how to approach male rhinoplasty

In a male patient in his early 20s presenting with a crooked nose, dorsal hump, and nasal obstruction, osteotomy, tip rhinoplasty using donor rib cartilage and septal cartilage, and columellar correction were performed.

Why the columella retracts and the alar rim looks pinched after rhinoplasty, and how autologous rib cartilage reconstruction is performed

In a patient presenting with columellar retraction and a pinched right alar rim following rhinoplasty with silicone and donor rib cartilage, the existing materials were removed and reconstruction of the septum and alar cartilage was performed using autologous rib cartilage.

Why the nasal bridge looks low and the tip looks bulbous: how to correct them alongside a retracted columella

To improve the low, bulbous nasal tip and retracted columella in a patient with no previous surgical history, autologous rib cartilage was used to reinforce the septum, elevate the nasal bridge and nasal tip, and correct the nasolabial angle.

An upturned nasal tip, a short columella, and an unnatural silicone bridge line from previous rhinoplasty were improved using autologous rib cartilage septal extension, custom-carved silicone implant replacement, and nasolabial angle correction.

Common questions before reading the cases

Is a retracted columella documented only in patients who have had prior rhinoplasty?

No. There are cases where a retracted columella and an unfavorable nasolabial angle were noted in primary rhinoplasty patients. Conversely, other records describe an inward-curled appearance and a depressed columellar scar from previous open surgery. Even when the term retracted columella is used, one can distinguish between conditions present congenitally and those documented alongside a history of previous surgery.

Which photos and descriptions are helpful to look at when the columella appears hidden?

One article separately details a columella that is barely visible on profile view, columellar retraction on oblique view, and an inward-curled columella on basal nostril views. Another case also records nostril asymmetry and depression around the alar base. Rather than comparing height from a single angle, read through the findings across the multiple views presented in the original notes.
