Bulbous nose and blunt tip: identifying areas of widened tissue

Even among patients who visited with a bulbous nose, the areas that appeared wide differed. In some cases, a patient whose nasal tip looked round and spread from the front also had columellar tilt and nostril asymmetry, while in others, both the nasal tip and bridge appeared wide along with a drooping tip.

Therefore, rather than showing a blunt tip in isolation, this collection is organized so you can examine bridge width, tip position, and nostril shape together. In one original article, the surgical plan addresses a spread and drooping tip, a shifted implant, and septal deviation separately. Even within results that appear slimmer overall, the specific areas addressed can vary.

Why the nasal tip turns up and the columella droops, and how septal extension is performed

A case where a short, upturned nasal tip and a drooping columella in an unoperated nose were corrected through septal extension. A strut was built using donor rib cartilage and septal cartilage, accompanied by columellar elevation and silicone implant placement.

Why noses deviate and bulbous tips persist after double jaw surgery, and how rib cartilage septal reconstruction is performed

This is a case of septal damage, a deviated nasal bridge, a bulbous nose, and nasal obstruction following double jaw surgery and two prior rhinoplasties. Autologous rib cartilage septal reconstruction, cartilage repositioning, lateral osteotomy, and functional nasal surgery were performed.

Why a high radix makes the nose look long and overly prominent

Following prior surgeries, a bulbous and drooping long nose with an excessively high radix and nasal obstruction was corrected using a closed approach. Scar tissue debridement, osteotomy, nasolabial angle correction, and septoplasty were performed concurrently.

Why the nasal tip can look bulbous and crooked after rhinoplasty with ear cartilage

This is a revision rhinoplasty case where the patient experienced a drooping tip, a deviated septum, and nasal obstruction after previous surgery. The septum was reconstructed using autologous rib cartilage, and silicone implant replacement along with rhinitis surgery was performed using a closed approach.

Why a operated nose deviates and nostrils become asymmetric after physical trauma, and how septal reconstruction is performed

A case where post-surgical trauma caused a septal tear, resulting in a crooked nose and nostril asymmetry. Septal reconstruction was performed using autologous rib cartilage, along with realigning the columellar central axis and replacing the implant.

Before-and-after comparison

Why a nose operated on twice looks long and drooping, and how septal reconstruction using rib cartilage is performed

A revision rhinoplasty case where the septum was depleted due to excessive cartilage use in prior surgeries. The septum was reconstructed using autologous rib cartilage, a cartilage fragment on the bridge was removed, and silicone implant replacement was combined with osteotomy.

Changes that occur when an implant is positioned high and deviated: methods for replacement and bulbous tip correction

A high, deviated silicone implant was replaced to lower the radix, and septal extension was performed using donor rib cartilage. Partial soft tissue resection, cartilage tie, and rhinitis surgery addressed the bulbous tip and nasal obstruction.

Why the nasal tip deviates to one side after rhinoplasty and methods for reconstructing its support structure

Septal cartilage damaged from previous surgeries was reconstructed using autologous rib cartilage, and the lower lateral cartilages were restored. Lateral osteotomy and rhinitis surgery were performed concurrently to correct the crooked nose and nostril asymmetry.

Why the nose looks short and the columella retracts after revision rhinoplasty, and how to reconstruct the septum

To correct septal cartilage loss from 3 prior surgeries, a short nose appearance, and a bulbous tip, the septum was reconstructed using autologous rib cartilage and the radix was lowered.

Why a dorsal hump looks prominent and how to correct it along with the nasal tip

Previously injected nasal filler was removed, the protruding dorsal hump was rasped, and lateral osteotomies were performed. The blunt nasal tip was elevated using autologous rib cartilage, and both the nasolabial angle and septum were corrected.

Why a multiply operated nose deviates and develops a widened tip, and how cartilage is reconstructed

Following three prior procedures, a deviated nose caused by septal damage and a missing right lower lateral cartilage was restored through autologous rib cartilage structural reconstruction, silicone implant replacement, nasolabial angle correction, and rhinitis surgery.

Why an upturned tip makes the mouth look more protruding and how to correct it

A bulbous, upturned nasal tip, the appearance of a protruding mouth, and nasal obstruction were addressed by removing columellar filler, reinforcing the nasal tip with autologous rib cartilage, placing a customized silicone implant, adjusting the nasolabial angle, and correcting a deviated septum.

Common questions before reading the cases

Can a bulbous nose and a wide nasal bridge be viewed as the same issue?

In the original articles, there are cases where tip spreading and bridge width are explained separately. In one patient, both the tip and bridge appeared wide, but a shifted nasal tip and nostril asymmetry were also documented. First, distinguish from the front photos whether the area that appears wide is the tip, the bridge, or both.

Why do you also evaluate the columella and nostrils in cases with a round nasal tip?

One revision rhinoplasty record notes that along with a round, spread tip, the columella was tilted to the left and nostril asymmetry was present. The round appearance of the tip alone did not fully capture the patient's concerns. Reviewing the nostril photos and preoperative findings in that article shows how structural width and central axis issues can overlap.
