Osteotomy and nasal bone correction before-and-after cases

When looking into osteotomy or nasal bone correction, first check whether the procedure aims to narrow a wide nasal bridge or to address crooked bones and asymmetry from previous surgery. The records include cases where osteotomy was performed for dorsal hump reduction and reshaping the nasal profile, as well as cases where asymmetry after previous surgery was corrected with a new osteotomy while adjusting the width of the nasal bridge.

In revision cases where very little nasal bone remained, the records also describe repositioning the bones on both sides and filling the gap with autologous rib cartilage. Therefore, not all nasal bone procedures should be understood simply as narrowing or shaving down the bone. Reviewing the preoperative bone shape, the presence of bone defects, and the concurrent use of cartilage or implants helps you interpret the changes in the photos more accurately.

Why rib cartilage reconstruction is needed when the septum is weak, even in primary rhinoplasty

A case of correcting a short, upturned nasal tip and a pinched nostril caused by congenital septal cartilage weakness. Autologous rib cartilage was used to reconstruct the septum and lower lateral cartilage, combined with lowering the nostril rim and lateral osteotomy.

Why a nose operated on twice looks long and drooping, and how septal reconstruction using rib cartilage is performed

A revision rhinoplasty case where the septum was depleted due to excessive cartilage use in prior surgeries. The septum was reconstructed using autologous rib cartilage, a cartilage fragment on the bridge was removed, and silicone implant replacement was combined with osteotomy.

Why a short, upturned nose makes the philtrum look long and septal extension graft

We extended the septum with autologous rib cartilage and corrected the nasolabial angle to improve an upturned nasal tip and a long philtrum. Lateral osteotomy, a custom silicone implant, and rhinitis surgery were performed concurrently.

Why the nasal tip deviates to one side after rhinoplasty and methods for reconstructing its support structure

Septal cartilage damaged from previous surgeries was reconstructed using autologous rib cartilage, and the lower lateral cartilages were restored. Lateral osteotomy and rhinitis surgery were performed concurrently to correct the crooked nose and nostril asymmetry.

Why implants show through and nostrils look M-shaped, and how to reconstruct them

Implant show-through at the nasal tip and thinned skin were reinforced with donor dermis, and foreign substances were removed. The nasal tip was reconstructed using autologous rib cartilage, along with dragon nose correction and lateral osteotomy.

Why a dorsal hump looks prominent and how to correct it along with the nasal tip

Previously injected nasal filler was removed, the protruding dorsal hump was rasped, and lateral osteotomies were performed. The blunt nasal tip was elevated using autologous rib cartilage, and both the nasolabial angle and septum were corrected.

Why a nose looks bulbous or pug-like, and how to refine both the nasal tip cartilage and nasal bones together

A blunt, pug-like nose, retracted columella, and wide nasal bridge were transformed into a slim, three-dimensional profile using autologous rib cartilage tip plasty, columellar lowering, lateral osteotomy, and cartilage reduction with suturing.

Common questions before reading the cases

Does having an osteotomy mean a silicone implant must also be used on the nasal bridge?

There are cases where osteotomy and bridge contouring were performed without adding an implant. Conversely, in revision cases with substantial nasal bone loss, the records show instances where the gap was filled with autologous rib cartilage and covered with a silicone implant. The decision to use an implant cannot be determined simply by the name of the procedure, osteotomy; one must consider the initial condition of the bone alongside what was actually performed.

Are there also records of correcting asymmetry resulting from previous nasal bone surgery?

There are cases where a new osteotomy addressed asymmetry from a previous surgery while adjusting the width and contour of the nasal bridge. You can also review separate records where bones were realigned and reinforced in cases with limited remaining bone. Because the condition of the remaining bone varies even under the same label of revision correction, comparing the preoperative evaluations is helpful.
