Defined and prominent rhinoplasty design cases

Even when patients say they want a defined, prominent nose, some hope for a clearer line while preserving naturalness, while others simply prefer a high bridge. In one case, a patient requested a natural yet slightly defined feel; in another, the patient wanted a high shape, but we explained that the surgical direction would be decided during surgery based on the existing nasal anatomy.

We have also included case notes where the postoperative outcome was described as natural yet defined, while taking a long philtrum and asymmetry into account. This collection does not imply that every requested height or shape was fully achieved as asked. Examining the patient's design goals, the structural conditions found during surgery, and the postoperative notes helps clarify what is meant by the word defined in each situation.

Why the nasal tip turns up and the columella droops, and how septal extension is performed

A case where a short, upturned nasal tip and a drooping columella in an unoperated nose were corrected through septal extension. A strut was built using donor rib cartilage and septal cartilage, accompanied by columellar elevation and silicone implant placement.

Why a narrow nasolabial angle with a dorsal hump makes the mouth appear protruding and how to correct it

This is a primary rhinoplasty case correcting an irregular dorsal hump and an acute nasolabial angle. Using a closed approach, we refined the dorsal hump, performed nasal tip plasty with autologous rib cartilage, reconstructed the septum, placed a silicone implant, and corrected the nasolabial angle.

Why the nasal tip can look bulbous and crooked after rhinoplasty with ear cartilage

This is a revision rhinoplasty case where the patient experienced a drooping tip, a deviated septum, and nasal obstruction after previous surgery. The septum was reconstructed using autologous rib cartilage, and silicone implant replacement along with rhinitis surgery was performed using a closed approach.

Why the nasal tip elongates and droops years after rhinoplasty, and how to correct it

A case involving a long, drooping nasal tip and nasal obstruction following previous rhinoplasty with a silicone implant and septal cartilage. The nasal tip was corrected using autologous rib cartilage, performed concurrently with septoplasty, inferior turbinate reduction, and synechia adhesiolysis.

Why nasal tip skin depresses and implants show through after inflammation, and how to reconstruct it

A case where repeated surgeries and inflammation led to depressed nasal tip skin and implant show-through. The damaged skin was revised, dermal and cartilage grafts were placed, the septum was reinforced with autologous rib cartilage, and the implant was replaced.

Before-and-after comparison

Why a nose operated on twice looks long and drooping, and how septal reconstruction using rib cartilage is performed

A revision rhinoplasty case where the septum was depleted due to excessive cartilage use in prior surgeries. The septum was reconstructed using autologous rib cartilage, a cartilage fragment on the bridge was removed, and silicone implant replacement was combined with osteotomy.

Changes that occur when an implant is positioned high and deviated: methods for replacement and bulbous tip correction

A high, deviated silicone implant was replaced to lower the radix, and septal extension was performed using donor rib cartilage. Partial soft tissue resection, cartilage tie, and rhinitis surgery addressed the bulbous tip and nasal obstruction.

Why an upturned tip makes the mouth look more protruding and how to correct it

A bulbous, upturned nasal tip, the appearance of a protruding mouth, and nasal obstruction were addressed by removing columellar filler, reinforcing the nasal tip with autologous rib cartilage, placing a customized silicone implant, adjusting the nasolabial angle, and correcting a deviated septum.

Why a nose looks bulbous or pug-like, and how to refine both the nasal tip cartilage and nasal bones together

A blunt, pug-like nose, retracted columella, and wide nasal bridge were transformed into a slim, three-dimensional profile using autologous rib cartilage tip plasty, columellar lowering, lateral osteotomy, and cartilage reduction with suturing.

Why nasal tip skin thins after multiple surgeries: autologous dermis reinforcement and reconstruction methods

We removed previous foreign materials and implants, reconstructed the nasal tip using autologous rib cartilage, reinforced the thinned skin with autologous dermis, recarved a silicone implant, and performed fat grafting between the eyebrows.

Why dorsal humps and drooping tips appear together, and how hump reduction and osteotomy are performed

A dorsal hump and drooping tip were smoothly refined through a closed approach using autologous rib cartilage tip plasty, hump reduction, lateral osteotomy, and nasolabial angle correction.

Why the nasal tip looks bulbous and spread out, and how septal extension is performed

A bulbous, spread-out nasal tip and deviated septum were refined with autologous rib cartilage septal extension, nasolabial angle correction, a silicone implant on the nasal dorsum, and functional nasal surgery.

Why nasal filler cannot correct a drooping tip and dorsal hump, and the reconstruction method after removal

To resolve a persistent drooping tip and dorsal hump deformity after filler and PDO thread lifting, foreign material was removed, followed by nasal tip reconstruction using autologous rib cartilage, a silicone implant on the nasal bridge, and septoplasty.

Why a nose appears low and how to build height using autologous rib cartilage

For a male patient who had received repeated filler injections for a low nasal bridge, foreign material was removed, followed by nasal tip rhinoplasty using autologous rib cartilage and the insertion of a customized silicone implant.

Common questions before reading the cases

Are these cases where the nose was always raised high just because the patient wanted a defined look?

No. In some cases, patients wanted a high, defined appearance, but we discussed the plan based on their existing nasal structure and what we would find during surgery. There are also records where patients preferred a natural look with a slightly defined touch. Since a patient's preferences reflect preoperative hopes, the actual materials used, the procedures performed, and the follow-up observations must be reviewed.

Are there cases where patients wanted both a natural and a defined look?

Yes, there are records of requests for a natural yet slightly defined, slim shape. There are also cases described as achieving a natural yet defined result while addressing a long philtrum and asymmetry. These two descriptions are not necessarily contradictory; reviewing the context shows whether the discussion was about height, width, or facial proportions.
