Deviated septum cases: a deviated airway and remaining cartilage

In records addressing septal deviation, nasal obstruction and external deviation often appear together. In a primary rhinoplasty patient, a dorsal hump, a crooked nasal bridge, and nasal obstruction were documented together, while in a revision patient, findings noted a deviated septum along with a depleted amount of remaining cartilage due to prior harvesting.

Check the condition of the cartilage described in the original records, even within the same term "deviation." In one case, a septum that was thinner and more deviated than expected was identified during surgery, requiring partial reconstruction followed by septal extension. Separating the steps taken to straighten the septum from those used to support or elevate the nasal tip clarifies the surgical sequence.

Why the nose looks long and the nostrils look pinched after multiple rhinoplasties

Lost lower lateral cartilage was reconstructed using autologous rib cartilage, and asymmetric, pinched nostrils were corrected. By lowering the radix, the appearance of a long nose was reduced, and surgery for rhinitis and nasal valve stenosis was performed concurrently.

Why contracture remains in multiply operated noses: Reconstructing the columella and support framework

Contracture and septal defect resulting from repeated surgeries were reconstructed using autologous rib cartilage. For the retracted columella, retroauricular free skin flap grafting and perichondrium grafting were performed to restore its shape.

Nasal obstruction from a severely deviated septum: why cartilage reconstruction is necessary

In a primary rhinoplasty patient, a severely deviated septum was reconstructed using autologous rib cartilage, and hypertrophied inferior turbinates were reduced. Dermis was grafted into the depressed alar region, and the nasolabial angle was adjusted.

Why nostrils become asymmetrical and the nose looks crooked, and how rib cartilage reconstruction is performed

Nostril asymmetry caused by a tilted columellar axis and loss of alar cartilage after Gore-Tex insertion was corrected through Gore-Tex removal, autologous rib cartilage alar reconstruction, and septoplasty.

Why an upturned tip makes the mouth look more protruding and how to correct it

A bulbous, upturned nasal tip, the appearance of a protruding mouth, and nasal obstruction were addressed by removing columellar filler, reinforcing the nasal tip with autologous rib cartilage, placing a customized silicone implant, adjusting the nasolabial angle, and correcting a deviated septum.

Why the nasal bridge becomes uneven after rhinoplasty, and how septal reconstruction using rib cartilage is performed

An uneven nasal bridge, damaged septal cartilage, and nasal obstruction were corrected by removing the existing silicone implant and filler, followed by autologous rib cartilage reconstruction, re-insertion of a customized silicone implant, and septoplasty with inferior turbinoplasty.

Common questions before reading the cases

Do cases of septal deviation correction mean the same thing as septal reconstruction cases?

They can be distinguished in the original text. One case notes that because the septum was thinner and more severely deviated than expected, partial reconstruction was performed using autologous rib cartilage. In another revision case, it is explained that almost no septal cartilage remained due to prior harvesting. It is helpful to distinguish between the condition of deviation itself and the specific correction or reconstruction performed to address it.

Why does nasal tip height also appear in septal surgery records?

One original record explains that after partially reconstructing the septum, septal extension was performed in that area using autologous rib cartilage to elevate the nasal tip. This is a case where the step addressing the condition of the septum and the step changing the nasal tip were connected in the same surgery. Review how documentation of nasal obstruction or deviation relates to cosmetic changes in the nasal tip.
