Implant-free rhinoplasty before-and-after cases

When looking into implant-free rhinoplasty, it is important to distinguish between not using silicone and not using any cartilage at all. In actual clinical records, there are cases where the nasal bridge was elevated without an implant by using donor rib cartilage and pieces of septal cartilage. The term implant-free does not necessarily mean that only one's own cartilage was used.

In dorsal hump cases where the nasal bones are naturally large, we explain both the process of smoothing the bone and how the transition between the nasal tip and nasal bridge was designed. Even within the topic of implant-free rhinoplasty, an augmentation procedure differs from a procedure that trims existing structures. Rather than simply noting a patient's desire to avoid implants, look closely at records that detail the actual surgical steps performed.

Why the nasal bridge deviates and the tip appears drooping: How to correct it using only cartilage without implants

For a patient who wanted primary rhinoplasty without artificial implants, lateral osteotomy was performed to straighten the deviated nasal bones, and donor rib cartilage and septal cartilage were used to reinforce the bridge and reshape the nasal tip.

Why an upturned nose and unilaterally pinched nostril occur, and how implant-free reconstruction is performed

An upturned nose and pinched nostril caused by multiple prior surgeries were corrected after removing artificial dermis, performing implant-free bridge augmentation with donor rib cartilage and septal cartilage, and using a skin-cartilage composite graft on the right nostril.

Why nostrils look different when tilting the head up and cause nasal obstruction: septal reconstruction

To correct nostril asymmetry, a short nose, and nasal obstruction that developed after previous implant-free rhinoplasty and alar rim lowering, autologous rib cartilage septal reconstruction and functional nasal surgery were performed.

An uneven nasal bridge after a dermal graft: causes and implant-free reconstruction

A case where a previously placed dermal graft had caused an uneven nasal bridge, alongside an unresolved bulbous nose and nostril asymmetry, which were corrected by completely removing the existing dermis and performing a comprehensive reconstruction using autologous rib cartilage.

Why the nasal dorsum appears depressed and the tip looks bulbous after dorsal hump reduction, and how to correct it without implants

To improve a bulbous nasal tip, residual hump sensation, and asymmetry without silicone implants, autologous rib cartilage was used to elevate the nasal bridge and dorsal line, combined with cartilage repositioning, cartilage binding, and nasolabial angle correction.

Common questions before reading the cases

Does implant-free rhinoplasty only use autologous rib cartilage?

The records include cases where the nasal bridge was elevated without implants such as silicone, using donor rib cartilage and pieces of septal cartilage instead. Therefore, the term implant-free alone does not mean that only autologous rib cartilage was used. Checking both the absence of artificial implants and the source of the cartilage will help you find cases that closely match your preferred material choices.

In implant-free cases, is there a difference between augmenting the nasal bridge and trimming a dorsal hump?

There are records of augmenting the nasal bridge with materials such as donor rib cartilage, as well as records explaining the transition between the nasal tip and bridge while trimming naturally large nasal bones and a dorsal hump. Because the initial nasal height and anatomical structure differ, it is difficult to compare results simply under the umbrella term of implant-free rhinoplasty. It is helpful to review both the preoperative anatomy and the actual materials used.
