Hello, I am Dr. Peter C. Kang.

Today, I would like to share a case of a patient who visited us concerned about a wide, blunt nose and a bulbous tip.

This was a meaningful case that went beyond simply refining the nose shape to transform the patient's overall facial impression.

🔍 Key symptoms

  • Wide and blunt nose

  • Nasal obstruction

  • Protruding mouth

💉 Preoperative analysis

Design consultation: front, right oblique, and profile views.

1. Front view

  • Flared nasal tip

  • Poorly defined junction between the alae and nasal tip

  • Deviated nose

2. Profile view

  • Low radix

  • Protruding mouth

  • Irregularity due to a minor dorsal hump

3. Nostrils

  • Nostril asymmetry

  • Flat columella

  • Flared nasal tip

Design consultation photos: left oblique view, profile view, and nostril view.

Nasal obstruction testing confirmed leftward deviation of the septal cartilage and bilateral inferior turbinate hypertrophy, leading to a diagnosis of a deviated septum and hypertrophic rhinitis.

🔧 Surgical plan and procedure

1. Functional improvement

  • Septoplasty

  • Radiofrequency submucosal resection of the inferior turbinate

2. Structural correction

  • Correction of the deviated nasal bridge via lateral osteotomy

  • Nasal bridge augmentation using a silicone implant

  • Septal extension using autologous rib cartilage

3. Nasal tip improvement

  • Partial excision of subcutaneous tissue to reduce thickness

  • Repositioning and suturing of lower lateral cartilage

  • Lowering the columella with autologous rib cartilage

🌟 Surgical results

1. Frontal changes

Before surgery / Day 0 immediately after surgery Photos taken immediately after surgery show swelling and may differ from the final result.

  • The previously wide nasal tip was refined into a sleeker shape

  • The previously undefined boundary between the alae and nasal tip became clear

  • The previously crooked nose was straightened

  • The overall facial impression transformed into a refined look

2. Profile changes

Before surgery / Day 0 immediately after surgery Photos taken immediately after surgery show swelling and may differ from the final result.

  • The low glabella was naturally elevated

  • The appearance of a protruding mouth was improved, softening the profile silhouette

  • Irregularities from the dorsal hump were smoothly corrected

  • A natural nasolabial angle was created, achieving a feminine, well-balanced aesthetic

3. Nostril improvement

Before surgery / Day 0 immediately after surgery Photos taken immediately after surgery show swelling and may differ from the final result.

  • The asymmetric nostrils were corrected into a balanced shape

  • The flat columella was naturally brought downward

  • The wide nasal tip was brought inward for a refined appearance

4. Overall improvement

  • Significant improvement in overall facial balance

  • Enhanced facial dimension with the formation of a natural nasal line

  • A noticeably softer impression due to the improved appearance of a protruding mouth

  • Transformation toward a feminine and sophisticated image

  • High functional satisfaction with the simultaneous resolution of nasal obstruction

💡 Closing remarks

Rhinoplasty is not simply a surgery to make the nose alone look pretty. As seen in this case, improving the shape of the nose has a major impact on the balance and harmony of the entire face.

In particular, improving the appearance of the protruding mouth played a decisive role in changing the overall facial image. The facial three-dimensionality was restored and the feminine image was accentuated, resulting in very high satisfaction for the patient.

Moving forward, I will continue to do my best to provide customized rhinoplasty that considers the harmony and balance of the entire face, rather than just simple reshaping of the nose.

I hope this case helps those who share similar concerns.

This was Dr. Peter C. Kang. Thank you.

Thank you.

Limitations and risks

When autologous rib cartilage is used, scarring and pain may remain at the donor site on the chest, and pneumothorax (pleural injury) can occur rarely. Over time, the grafted cartilage may warp or undergo partial resorption.

When the septum is corrected at the same time, nasal obstruction may persist or recur, and in rare cases, septal perforation or changes in smell may occur.

Common potential risks include infection, bleeding, asymmetry, scarring, skin necrosis, and insufficient improvement; results vary depending on individual skin condition and previous surgical history.
