Hello.
I am Dr. Peter C. Kang, medical director.
Today, I would like to introduce the case of a patient who underwent a 4th revision rhinoplasty.
To correct the pinched nose, alar rim elongation was performed; to address the long nose, the radix starting point was adjusted downward, and lowering the columella was carried out.
Comparison of before-and-after photos
Profile view:
Before surgery:
The radix starting point is low.
A slight dorsal hump is observed.
There is a slight depression at the junction between the nasal bridge and the nasal tip.
The nasal tip is slightly drooping.
The nasolabial angle is rather small.
Immediately after surgery (Day 0): Immediate postoperative photos show swelling and may differ from the final result.
The radix starting point has been raised, forming a more natural line.
The dorsal hump has been corrected, presenting a smoother profile.
The transition between the nasal bridge and the nasal tip has been smoothly refined.
The nasal tip is slightly elevated, creating a more balanced appearance.
The nasolabial angle has slightly improved, appearing more natural.
Oblique view:
Before surgery:
The nasal bridge line is not smooth and has a slight irregularity.
Overall, the nose has a slightly drooping appearance.
Immediately after surgery (Day 0): Immediate postoperative photos show swelling and may differ from the final result.
The nasal bridge line has become smoother and more natural.
The overall profile of the nose has improved, showing a more balanced appearance.
Front view:
Before surgery:
The nose is slightly deviated to the right.
The radix starting point is somewhat low.
The nasal tip is blunt and slightly spread.
The nostrils appear slightly asymmetric.
Immediately after surgery (Day 0): Immediate postoperative photos show swelling and may differ from the final result.
The nasal deviation has been corrected, presenting a straighter appearance.
The radix starting point has been slightly raised, creating a more natural line.
Definition in the nasal tip has improved, making it look less blunt.
Symmetry of the nostrils has improved.
Basal (nostril) view:
Before surgery:
Nostril asymmetry is observed.
The nasal base appears slightly wide.
The nasal tip is rather blunt.
Immediately after surgery (Day 0): Immediate postoperative photos show swelling and may differ from the final result.
Symmetry of the nostrils has improved.
The width of the nasal base has slightly decreased, appearing more natural.
Definition in the nasal tip has improved, making it look less blunt.
Closing remarks
Overall, through closed rhinoplasty, the nasal deviation was corrected, the lines of the nasal bridge and nasal tip were improved, and overall balance and symmetry were enhanced. Even immediately after surgery, it shows a natural result.

Limitations and risks
Common potential risks include infection, bleeding, asymmetry, scarring, skin necrosis, and insufficient improvement; results vary depending on individual skin condition and previous surgical history.
