Hello, I am Dr. Peter C. Kang.

Today, I would like to introduce the case of a patient who wanted to refine a nose that looked spread out after receiving nose filler into a slim, well-defined shape.

Front, left profile, and oblique views during design consultation

📍 Surgical history

  • March 2020: Nasal filler injection (never dissolved)

🔎 Initial consultation findings

Design consultation: right oblique, profile, and basal (nostril) views

  1. Front view

  • Nose appears wide and spread out

  • Crooked nasal shape

  • Alar asymmetry

  • Bulbous nasal tip

2. Profile view

  • Dorsal hump shape

  • Protruding mouth findings

  • Acute nasolabial angle

  • Bulbous nasal tip

  • Columellar line requires improvement

3. Nostrils

  • Asymmetry with the left nostril being larger

  • Overall imbalance

4. Nasal obstruction

  • Rhinitis symptoms

✨ Patient's requests

  • Improvement of a wide nose

  • Overall slim contour

  • Improvement of nasal obstruction

💉 Surgical procedure details

  1. Structural improvement

  • Correction of the widened and deviated nasal bridge using lateral osteotomy

  • Nasal tip elevation using autologous rib cartilage

  • Rasping of the dorsal hump

2. Shape improvement

  • Improvement of protruding mouth through nasolabial angle correction

  • Formation of a refined columellar line

  • Natural adjustment of the width

3. Balance improvement

  • Nasal sill reduction

  • Correction of nostril asymmetry

  • Securing overall harmony

4. Surgery to improve nasal obstruction

  • Septoplasty

  • Radiofrequency submucosal resection of the inferior turbinate

For this type of case, the main goal was to refine a nose that looked spread out following filler injections into a sleek, slim contour. Because the septal cartilage was deviated and small, autologous rib cartilage was utilized, and lateral osteotomy was used to naturally bring the widened nasal bridge together. In particular, by rasping the dorsal hump and performing nasal sill reduction, we were able to achieve the slim profile line the patient desired. Correcting the nasolabial angle also improved the appearance of the protruding mouth, yielding an overall harmonious outcome.

This was Dr. Peter C. Kang. Thank you.

Thank you.

Limitations and risks

When autologous rib cartilage is used, scarring and pain may remain at the donor site on the chest, and pneumothorax (pleural injury) can occur rarely. Over time, the grafted cartilage may warp or undergo partial resorption.

When the septum is corrected at the same time, nasal obstruction may persist or recur, and in rare cases, septal perforation or changes in smell may occur.

Common potential risks include infection, bleeding, asymmetry, scarring, skin necrosis, and insufficient improvement; results vary depending on individual skin condition and previous surgical history.
