Hello, I am Dr. Peter C. Kang.
Today, I would like to introduce the case of a patient who, after a long period of contemplation, decided to undergo surgery to address spread nasal filler and a long nose appearance.
📍 Procedure history
• 5 years ago: Nasal filler injection (never dissolved)
🤒 Current symptoms
• Nasal obstruction
• Frequent rhinorrhea
• 1 year ago: Diagnosed with sinusitis
• History of 1 month of medical therapy at an ENT clinic
✨ Patient's requests
• Improvement of spread nasal filler
• Dorsal hump correction
• Correction of a long-looking nose
• Natural and harmonious result
• Desire to maintain nasal height
💉 Surgical procedure details
1. Removal surgery
• Safe removal of existing nasal filler
• Smoothing and reduction of the dorsal hump
2. Structural improvement
• Natural adjustment of the seemingly long nasal length (lowering the radix, elevating the nasal tip)
• Septoplasty
• Overall natural proportions
3. Functional improvement
• Septoplasty
• Surgery to reduce the volume of the hypertrophied inferior turbinates
• The nasal cavity is expected to widen in a balanced manner, preventing sinusitis from worsening
✅ Surgical results
1. Front view
• Improvement of the spread nasal bridge line
• Clean, refined nasal bridge line
• Natural width
• Correction of the blunt nasal tip
2. Profile view
• Natural lowering of the glabella starting point
• Correction of the long nose appearance
• Correction of the blunt and drooping nasal tip
3. Function
• Improvement in nasal obstruction and rhinorrhea is expected
• Sinusitis symptoms are expected to decrease
👨⚕️ Specialist's assessment
In this surgical case, which was decided after 10 years of careful deliberation, both the patient's expectations and concerns had to be addressed. In particular, because the patient was concerned about a loss of nasal height after filler removal, the focus was placed on safe filler removal while maintaining a natural height. By preventing the recurrence of sinusitis while correcting aesthetic aspects such as the dorsal hump and long nose, a satisfying outcome was achieved in both function and aesthetics.
This was Dr. Peter C. Kang. Thank you.

Limitations and risks
When foreign materials such as injected fillers are removed and reconstructed, contracture may recur, nasal height and length may decrease, and staged surgery may be necessary depending on the skin condition.
When the septum is corrected at the same time, nasal obstruction may persist or recur, and in rare cases, septal perforation or changes in smell may occur.
Common potential risks include infection, bleeding, asymmetry, scarring, skin necrosis, and insufficient improvement; results vary depending on individual skin condition and previous surgical history.
