Front, left profile, and oblique views during design consultation

Hello, I am Dr. Peter C. Kang.

Today, I will introduce the case of a patient who underwent reconstruction for bulbous nose correction and relief of nasal obstruction.

📍 Initial consultation findings

1. Front view

  • Wide nose

  • Low nasal bridge

  • Overall lack of three-dimensional definition

2. Profile view

  • Bulbous nasal tip

  • Protruding mouth findings

  • Low nasal bridge height

3. Nostrils

  • Asymmetry between the left and right nostrils

  • Broad, spread-out nasal tip

  • Lack of overall balance

✨ Patient's requests

  • Improvement of nasal obstruction

  • Defined nasal line

  • Bulbous nose correction

💉 Surgical procedure details

1. Bulbous nose correction - Step one

  • Partial removal of subcutaneous tissue to adjust nasal tip skin thickness

  • Creation of a natural nasal tip shape

  • Reduction of nasal tip width

2. Bulbous nose correction - Step two

  • Repositioning of the lower lateral cartilage

  • Meticulous cartilage suturing

  • Stabilization of the overall structure

3. Bulbous nose correction - Step three

  • Adding three-dimensional definition by adjusting nasal tip height

  • Achieving a natural slimness

  • The most crucial step in bulbous nose correction

For this type of nose, the most crucial part was bulbous nose correction. Three steps were carried out to correct the bulbous tip: first, partial removal of subcutaneous tissue to reduce nasal tip skin thickness; second, repositioning of the lower lateral cartilage and cartilage suturing; and third, adjusting the nasal tip height to impart three-dimensional definition.

The third step is particularly vital, as the core of bulbous nose correction ultimately lies in raising the nasal tip naturally so that it appears slender. Through this comprehensive approach, we were able to achieve both the glamorous nasal contour desired by the patient and functional improvement.

Limitations and risks

When the septum is corrected at the same time, nasal obstruction may persist or recur, and in rare cases, septal perforation or changes in smell may occur.

Common potential risks include infection, bleeding, asymmetry, scarring, skin necrosis, and insufficient improvement; results vary depending on individual skin condition and previous surgical history.
