Hello, I am Dr. Peter C. Kang.

Today, I would like to introduce a customized rhinoplasty case of an international patient who visited with very specific requests.

Design consultation: front, left oblique, and profile views.

🔍 Surgical history

  1. 2012: Rib cartilage rhinoplasty (Shanghai)

  2. 2020: Ear cartilage rhinoplasty (Jakarta, Indonesia)

  3. 2021: Donor rib cartilage rhinoplasty (Korea)

  4. May 2024: Silicone implant + autologous rib cartilage + paranasal augmentation (Shanghai)

The patient's specific requests

Right profile, oblique, and nostril views during design consultation.

  • Gently lower the retracted columella in a rounded contour

  • Avoid making the columella excessively long

  • Improvement of nostril asymmetry

  • Desire to maintain the existing silicone implant (no changes wanted to the nasal dorsum and bridge)

I was able to clearly understand the ideal nasal shape the patient hoped to achieve.

Detailed correction plan

  1. Naturally lower the columella

  2. Improvement of nostril asymmetry

  3. Excise the triangular scar tissue below the left columella

  4. Improve the front-view nostril show on the right side

Surgical procedure details

  • Columellar correction using donor rib cartilage

  • Nostril extension by grafting donor rib cartilage onto the right lower lateral cartilage

  • Excision of the triangular scar tissue beneath the left columella

  • Correction of nostril asymmetry

🌟 Surgical results

  • Nostril asymmetry improved on the front view, and the excessive visibility of the right nostril was naturally corrected

  • The previously retracted columella was naturally improved

  • Improved nostril asymmetry

  • The triangular scar tissue beneath the left columella was excised

💡 Closing remarks

We aim for customized rhinoplasty that reflects the patient's needs as closely as possible. As seen in this case, we carefully analyze the patient's specific requests and do our best to realize them thoroughly.

Just as every patient has unique facial features and desired outcomes, rhinoplasty requires a customized approach tailored to each individual. NOSELAB will continue to accurately identify patients' needs and remain a trustworthy clinic dedicated to achieving them.

This was Dr. Peter C. Kang. Thank you.

Limitations and risks

When using donor rib cartilage, long-term follow-up is necessary because the grafted cartilage may resorb or warp.

Common potential risks include infection, bleeding, asymmetry, scarring, skin necrosis, and insufficient improvement; results vary depending on individual skin condition and previous surgical history.
