Hello, I am Dr. Peter C. Kang.
Today, I will discuss surgery that naturally refines a bulbous and drooping nose, a case commonly seen around us.
Preoperative design
The patient wanted to correct the drooping appearance and refine the bulbous nasal tip into a well-defined shape, but preferred not to change the tip height significantly and did not want it to look sharp. Because of the volume in the tip, a slight increase in height is inevitable, but we entered the operating room with a plan to perform the surgery at as natural a height as possible.
Additionally, the patient's nose had a C-shaped deviation from the front, so we decided to correct this part as well.
Before surgery vs. immediately after surgery
Let us compare the preoperative photos with the photos immediately after surgery (Day 0).Immediate postoperative photos show swelling and may differ from the final result.
The most noticeable change is that the drooping appearance is completely gone. The nasal tip is well-defined while maintaining a soft, rounded contour, and the nasal bridge line has become smooth.
You can see that the area where the nasal tip appeared depressed has significantly improved.
The deviation visible from the front has improved considerably, and the nasal tip, which once appeared bulbous, has transformed into a much sleeker shape.
Looking at the nostril photos, the asymmetry where the right nostril was larger and had a different shape compared to the left has been substantially improved.

My thoughts on rhinoplasty for a bulbous and drooping nose
Bulbous and drooping noses are very commonly seen around us. When I see a naturally refined result near the end of surgery, I often feel quiet satisfaction. Because this patient was concerned about drastic changes, I paid even closer attention. Surgery that addresses shortcomings without a major height change is truly demanding. Of course, during the recovery period, the nose will appear higher and more exaggerated than in the immediate photos, but by around 6 months, it settles into an appearance close to these immediate postoperative photos.
For a bulbous and drooping nose, even a subtle modification can bring about a significant change in overall impression. I hope this helps those who are concerned about a bulbous and drooping nose.
This was Dr. Peter C. Kang. Thank you.
Limitations and risks
Common potential risks include infection, bleeding, asymmetry, scarring, skin necrosis, and insufficient improvement; results vary depending on individual skin condition and previous surgical history.
